(New) Implementation and outcomes of a telehealth centre in a tertiary hospital (THINK)

Objective
Telehealth support serves as a potential means of supporting patients in care transitions. Moreover, the utility of telehealth is expected to grow given its value in enhancing access to care.
The primary objective of the THINK study is to evaluate the implementation of the telehealth support centre at Yishun Health to identify the barriers and facilitators of implementation. The secondary objective is to study its effectiveness in terms of patient outcomes, resource utilisation and cost benefit. A subgroup analysis of the aged patient group will be conducted in view of older adults’ unique needs and perceptions towards telehealth.
