Key Findings
Geriatric Service Hub (GSH) is a novel multi-disciplinary care model that adopted a collaborative approach bringing together different healthcare service providers for early identification and delivery of targeted interventions for frail community-dwelling older adults in Singapore.
Given that the GSH is a complex intervention with multiple interacting components, a mixed-methods approach will be employed to provide information on its implementability and effectiveness, which can support programme planners and policymakers in their decision-making.
