Advance Care Planning Quality: A Learning Journey of Continuous Improvement

Advance Care Planning (ACP) is an ongoing process and conversation that support individuals and families at any stage of health or illness, in preparation for the future. It is necessary for individuals to understand, communicate and document their values, goals, and priorities regarding future care plans before losing mental capacity. Practicing quality ACP is challenging. Multiple barriers have been identified at different stages of the ACP process such as the low introduction and referral rates, unreadiness among individuals and family members, lack of confidence among ACP facilitators, and poor documentation.
The webinar aims to share and discuss inspiring works from the ACP Quality Implementation (QI) teams across various institutions to motivate others to embark on this journey of continuous improvement. It untangles commonly faced challenges and proposes potential solutions that could be applicable in the audience’s context. Lessons learnt from these QI journeys regardless of outcomes would be shared in the spirit of collective learning.
Knowledge translation and behavioural science principles were applied in the development of the above QI projects. The audience will be able to appreciate the role of both disciplines in the practice of ACP and healthcare in general.
This event is organised by Geriatric Education and Research Institute (GERI), in collaboration with the Agency for Integrated Care (AIC). The Knowledge to Practice Series (KPS) is conducted regularly by GERI, as a knowledge translation initiative that brings together various stakeholders, to share research findings and exchange knowledge, to enhance eldercare practice and policy formulation.
